Registered, Excepted and Branch of a Charity Form

This form is to be used to allow organisations that are Registered, Excepted, or a verified Branch or affiliate of a charity to provide evidence of their eligibility status (e.g. unique charity registration number or documentation confirming eligibility.

We require a letter signed by a trustee and on letterhead of the registered charity sent to charityvalidation@lloydsbanking.com titled with account name and confirmation of below information.  We will not change your product until we have received any required evidence and verified your status.
